Call for Nominations - Lourdes Health Foundation Healing Spirit Award
Last year Lourdes Health Foundation was honored to present Dr. Rodrigo Santos with the first annual “Healing Spirit Award.” Once again we are seeking nominations for this prestigious award, which will be presented at The Dance 2009 – Broadway on …Broad annual black tie fund raising event on April 4, 2009 at the Ritz Carlton Philadelphia.
You can access the nomination form below. Please take a moment to nominate a Physician that you feel has made a difference in the lives of the patients they serve. Completed nomination forms should be returned to Felicia DiMattia, Director of Special Events, via email (dimattiaf@lourdesnet.org) or via fax at 856-382-1782. The deadline for nominations is November 1, 2008.
